Obesity Reduction and help for at Risk Children

ZIAD Healthcare for the Underserved Inc. has instituted a new program to help reduce obesity, one of the countries major health problems. It is also interested in the good work martial arts schools do on character building for at risk children.

We have implemented a sliding scale program for low income disadvantaged persons to be able to avail themselves to the good training martial arts schools provide.

The program works simply in the following manner:

  1. The Martial Arts School signs up to be an active member of the program by completing a simple on line form or by sending a hard copy to PO Box 112 Caro, Michigan 48723.
  2. Once in the program ZIAD will conduct a campaign and direct interested students that qualify for the program to you for training. This will be done by fax or mail referral form.
  3. In addition to your enrollment form you send back via fax or mail a simple ZIAD form completed by the student.
  4. You charge your student for lessons based on the sliding scale income program.
  5. Example: If a student falls into the family income bracket that is 20% then you would collect 20% of your normal fees from the student as payment for lessons. The difference is your contribution.

  6. ZIAD is applying for grants to fund various other costs of training(uniforms, testing, etc…) and you will be part of this program and receive a portion of funds as decided by our board from time to time.
